We are looking to recruit a self-motivated and enthusiastic 0.75WTE clinical pharmacist (band 7/8a depending on experience) for this exciting new role to develop medicines optimisation services within our Primary Care Network. You will report to the Clinical Director for the network.
As an experience pharmacist you will be based in the St Thomas Medical Group. We are a large innovative forward-thinking practice, over several sites, with a wide mulit-disciplinary team approach to Primary Care. You will be working as part of a wider team alongside GPs and nurses; you will provide support to practice clinical and non-clinical staff to respond to prescribing and medicines queries, medicines reconciliation on transfer of care and long term conditions management. You will carry out patient medication reviews as well as supporting medicines safety systems, quality improvement and QOF process within the practice IT system.
You will demonstrate courageous and critical thinking, and be a caring, compassionate and committed clinical pharmacist. Acting within your professional boundaries you will provide support for the patients of the practice as part of an integrated team. In return you will be supported to develop the medicines optimisation agenda, deliver high quality care and advance your clinical pharmacist skills.
You will promote safe, competent clinical decision-making and provide expert care for our patients within the general practice setting. You will be an effortless communicator and work collaboratively with the whole general practice team to meet the needs of patients, support the delivery of policy and procedures and adapt to the requirements of patients, as needs change.
In addition to enrolment on the CPPE Primary Care Pharmacy Education Pathway this unique PCN role offers the opportunity to work collaboratively with the Royal Devon and Exeter NHS Foundation Trust established community services pharmacy team to promote medicines optimisation right across the patient pathway. Depending on experience, you will be encouraged to contribute to the development and training of the Trust band 6 clinical pharmacists rotational programme to support the future pharmacy workforce and promote integrated patient care. You will also have access to senior pharmacist professional mentorship via the Trust pharmacy team. You should be an independent prescriber or expect to work towards this qualification in the PCN pharmacist role.
The final working pattern will be discussed with successful candidates. Hours are negotiable. Pay is based on AfC scales, depending on qualifications and experience.
